By submitting this form, I hereby grant to Step Up For Students and its affiliates the irrevocable and unrestricted right to create, use and publish photographs or videos of me and/or my child/children listed below for the below media relations, publications, electronic reproductions and/or promotional materials or any other purpose and in any manner or medium (photo/still, video, sound). In addition, I grant my permission to alter the same without restriction, and to copyright the same. I hereby release the videographer, cinematographer, photographer and Step Up For Students and its affiliates from all claims and liability relating to said images and videos. I also give consent to Step Up For Students to use my written word or my child’s written word. Step Up For Students and its affiliates may use such material until I revoke this authorization via writing and submission to Step Up For Students, and Step Up For Students acknowledges receipt.
I grant the right to publish my child(ren)’s image, written word, or voice for the following reasons, including but not limited to:
- Print materials: flyers, brochures, annual reports, postcards, newsletters, advertisements in newspapers/magazines, billboards
- Web materials: web banners on various websites, e-mails, online advertisements, Step Up For Students website, Twitter feed, Facebook pages, Instagram, YouTube and other social media channels
- Television: public service announcements, advertisements
- Radio: public service announcements, advertisements
